Learning Objectives

Improve adherence to evidence-based clinical guidelines for geriatric conditions in practice.
Develop evidence-based plans to diagnose, treat and manage common clinical conditions, chronic diseases and common neurodevelopmental and behavioral health conditions seen in primary care.
Implement tools to manage chronic care in practice and help patients control chronic illnesses, which may include practice transformation and/or quality improvement activities.
Demonstrate the ability to communicate effectively with the patient to ensure that diagnosis and treatment recommendations are understood.
Recognize when to refer to, or consult with, other specialists to provide optimal patient care.
