Stressful life events or psychological trauma? Which treatment is most suitable?
The diagnosis and therapy of trauma consequence disorders are challenging and complex. Psychotherapists: are increasingly confronted with patients in whom post-traumatic stress disorder is suspected at least as a comorbidity, often without clear ideas about what trauma-focused treatment should actually involve.
All the more important is the careful distinction between the processing of non-specific stressful life events and psychological traumatisations. The present additional module is intended to help sharpen this differentiation as a supplement to the OPD-3 manual, as this has a significant impact on the selection of individually suitable psychotherapy.
The module also expands the assessment of psychodynamic foundations to include the effects of traumatic events on psychodynamics. It takes into account how individual psychodynamic prerequisites influence the perception and processing of stressful life events and thus protect against a subsequent disorder or promote the development of these.
The diagnostic process takes place in two stages: first after OPD-3, the detection of the psychodynamic initial situation before the traumatic event, followed by a second diagnostic step to operationalize the effects of the event using this module. This allows the distinction between specific symptomatology and psychodynamics, the identification of resources and vulnerable factors, as well as the early detection of therapy inhibitors.
Concise case examples illustrate the potentials of trauma-focusing psychotherapies, but also the need to carefully check in patients with a low integrated psychological structure whether trauma-focused treatment is only indicated after stability.
