interRAI Home Care (HC) Assessment Form and User’s Manual, Canadian English Edition, 9.1.3 (PDF)

The Canadian English edition of the interRAI Home Care (HC) Assessment System is designed to be a user-friendly, reliable, person-centered system that informs and guides comprehensive planning of care and services for elderly and disabled persons in community-based settings around the world. It focuses on the person’s functioning and quality of life by assessing needs, strengths, and preferences. It also facilitates referrals when appropriate. When used on multiple occasions, it provides the basis for an outcome-based assessment of the person’s response to care or services. The interRAI HC Assessment System can be used to assess persons with chronic needs for care, as well as with post-acute care needs (e.g., after hospitalization or in a hospital-at-home situation).
This book contains introductory material, an item-by-item guide to completing the assessment form, and the assessment form itself. The Canadian Edition has been customized for Canada, and the English language used in this publication is native to Canada.
About the Additional Materials
This book (published in 2019, version 9.1.3) includes additional materials that were not part of the previous edition (published 2012, version 9.1.2). These additional materials begin with an Errata and Addenda document that lists changes, updates, or additions to version 9.1.2 of the HC manual and assessment form. Following the Errata and Addenda document is a revised assessment form in which the errata/addenda have been implemented. Please note that this revised assessment form (version 9.1.3) is included here in addition to the original version 9.1.2.
