Together, these manuals describe how to score and interpret a complete assessment system for older people in the acute hospital environment. This system supports either “whole of episode” assessment for acute geriatric and medical services (pre-morbid, admission, review and discharge), or geriatric consultation (pre-morbid and current) formats for assessment and care planning. The system is designed for use by medical practitioners, nurses, or multi-disciplinary teams.
