The release of this Fascicle represents a shift in the diagnosis of lower genital tract lesions, moving from a primarily morphologic approach to an integrated assessment that incorporates microscopy and molecular biology. Additionally, the ability to integrate these diagnostic methods with clinical data for patient management has improved. With the introduction of prophylactic vaccines for HPV infections and cervical cancer precursors, pathology is also taking on a new public health role in disease monitoring, which is expected to expand over time. The recognition that cervical cancer is becoming increasingly preventable brings both a sense of achievement and a new goal of cancer prevention in underdeveloped nations.

While the Fascicle remains concise in order to serve as an atlas rather than a comprehensive textbook, it has been extensively updated to reflect changes in the field since the previous edition was published in 1992. The number of color illustrations has nearly doubled, and the text and photomicrographs emphasize the importance of immunohistochemistry and molecular in situ hybridization as complementary tools to morphology for routine clinical diagnosis. A binary classification (low-grade and high-grade squamous intraepithelial lesion) for histology, similar to TBS for cytology, has replaced the four-tiered dysplasia/CIS system and the three-tiered CIN system. This Fascicle will serve as a valuable resource for pathologists, residents, and students for years to come.
